What to Bring
To ensure a smooth check-in process, please be sure complete your mobile check-in prior to your visit.
If you don’t complete your mobile check-in, you will need to bring your health insurance card, photo ID (e.g., driver’s license or passport), list of current medications, including supplements and over-the-counter medications, and any relevant medical records or test results, if not already in our system
Referrals
When specialized care is needed, we collaborate seamlessly with Community Care Physicians’ trusted network of specialists. This connection ensures that your care remains coordinated, comprehensive, and personalized—without missing a step. Our practice is fully integrated with CCP’s electronic health record system, which allows our providers immediate access to your complete medical history. This enhances communication and helps us deliver the most efficient, informed care possible.
Co-payments and Account Balances
Payment is expected at the time of service. If your insurance requires that you pay a co-payment, they require us to collect it. For your convenience, we accept cash, checks, most major credit cards, as well as Apple Pay and Google Pay.
Confidentiality Privacy & HIPAA
Our Notice of Privacy Practices is available on our website, in our offices, and you will be asked to acknowledge the policy during the intake process for our office.
